General Dynamics Information Technology, Inc. seeks a Case Manager to provide comprehensive clinical support to DoW beneficiaries, coordinating medical and non‑medical resources and ensuring smooth transitions between levels of care.

You will assess needs, develop individualized plans, coordinate services, and evaluate outcomes to promote optimal health, function, and readiness within multidisciplinary teams.

Competenze

• Minimum 2-year RN degree or Master’s in social work with CSWE licensure.
• Active independent social work license (LCSW or equivalent) preferred.
• Strong coordination, facilitation and communication skills.

Mansioni

• Coordinate comprehensive healthcare and support services for personnel and families.
• Develop individualized care plans and track progress toward goals.
• Oversee discharge planning and navigation of care transitions.
• Collaborate with multidisciplinary teams to optimize readiness and function.
