Clinical Case Manager, Behavioral Health

Position Overview

This Clinical Case Manager position with My Care of Ohio is field-based and open to candidates in Ohio, specifically from Butler, Clermont, Hamilton, or Warren County. Candidates must hold a Licensed Independent Social Worker (LISW) license for Ohio.

The role involves applying advanced clinical judgment to facilitate member health and behavioral healthcare, assessing and planning care, collaborating with providers, and coordinating psychosocial services. The goal is to optimize outcomes through resource application and cost-effective strategies.

Key Responsibilities
  1. Conduct comprehensive assessments of member needs and eligibility, evaluating benefit plans and available services.
  2. Develop strategies to mitigate risk factors and address complex clinical issues impacting care.
  3. Perform crisis interventions for members experiencing behavioral health or medical crises, referring to appropriate providers, and following up on service delivery.
  4. Adapt clinical guidelines and procedures to meet assessment needs, ensuring proper benefits administration.
  5. Collaborate with healthcare professionals to influence member care and outcomes.
  6. Coach members on healthcare decisions and healthy lifestyle choices, promoting active participation in their care.
  7. Train and mentor junior colleagues on techniques and responsibilities.
