Compassion Health Care is seeking a dynamic individual for a Clinical Care Coordinator whose primary role is to work directly with one of our Primary Care Providers at our new Madison location. Operational Support & Management Coverage:
Provides management-level operational support and oversight to ensure the safe and efficient operation of the facility. Responds to facility or environmental emergencies, operational outages or disruptions, staff emergencies, patient volume or scheduling concerns requiring additional resources, and situations requiring management assistance or de-escalation, including concerns involving irate or escalated patients. Coordinates vendor access and activities in collaboration with site management and provides additional support as needed. Conducts facility rounds, monitors patient flow and overall operational readiness, and remains available to staff for questions, concerns, and immediate operational needs.
As a Clinical Care Coordinator at Compassion Health Care, you will work as part of a healthcare delivery team serving patients of all ages. The Clinical Care Coordinator utilizes the nursing process, including assessment, planning, implementation, evaluation, and recording of patient care and patient education. The position provides support to a Primary Care Provider by anticipating needs and providing assistance in the provision of care. In addition, the Clinical Care Coordinator coordinates services with other members of the health-care team to provide patient-centered care. The role practices within the scope of the North Carolina Nurse Practice Act and/or Compassion Health Care’s policies, procedures, and standards. Clinical Care Coordinators may also perform other duties across the organization as their skills and training permit and may be cross-trained in other areas to accomplish operational objectives.

Diploma or associate degree from a CMA training program is required; clinical experience in a primary care setting is preferred—current/valid CMA certification required. Basic Cardiac Life Support (BCLS) is required. Basic computer skills required; experience with Electronic Medical Record preferred.
The selected candidate must meet CHC’s employment eligibility requirements, including but not limited to, successfully completing pre-employment drug testing and criminal background screening.
