Clinical Care Coordinator III (RN) - Endocrine

Position Summary

Reporting to the Nurse Manager, the Clinical Care Coordinator III (RN) assists healthcare providers with patient care, health education, EMR management, and clinical protocols. Responsibilities include medication administration, point of care testing, patient education, and coordination of care across various settings. The role involves working closely with the care team to ensure excellent patient outcomes and may require travel for training or review purposes.

Responsibilities

  • Data collection and input into EMR
  • Developing patient care plans, managing medications, and administering immunizations
  • Executing nursing interventions and managing urgent situations
  • Coordinating diagnostic tests and reviewing results
  • Managing communications and ensuring infection control compliance
  • Participating in or leading health initiatives and training staff

Minimum Qualifications

  • Bachelor of Science in Nursing or equivalent
  • At least 4 years of related experience, including endocrinology or diabetes care
  • Current NY RN license, BLS certification

Preferred Qualifications

  • Experience in physician practice, CDCES, BC-ADM, or Endocrine Nurse Certification
