Job summary

Unison Health in Toledo, OH seeks a clinical assessments role to engage referrals and coordinate services for children and families. You will work with community partners, provide crisis intervention, and document interactions in an electronic health record while maintaining strict confidentiality.

The position requires high-quality customer service, strong communication, and the ability to manage a small caseload with fidelity to program policies.

Qualifications

• Must obtain certification as a CANS assessor after hire.
• Knowledge of mental illness including the SED population and crisis management.
• Flexible schedule including evenings and weekends.
• Proven ability to build therapeutic relationships with clients.

Responsibilities

• Outreach and engagement of clients for the OhioRISE program.
• Warm hand-off to the Care Coordinator.
• Document contacts and maintain records per policy.
• Maintain client confidentiality per federal guidelines.
• Participate in team meetings, supervision, and training.
