A bit about this role: The Clinical Associate is an integral part of our interdisciplinary virtual and home-based medical practice called Devoted Medical (DM). DM delivers multiple clinical services to our Devoted Health members, including wellness visits, transitions of care, longitudinal visits for complex members, and on-demand urgent care visits. The Clinical Associate acts as a liaison between our healthcare services and Devoted members, working closely with clinicians to provide high-touch telehealth visits and support the clinical team by identifying care gaps and coordinating across fragmented systems.
Work Schedule
- Monday - Friday 8:30 AM - 5:00 PM ET (or 9:30 AM - 6:00 PM ET for EST/CST candidates)
- Monday - Friday 10:00 AM – 6:30 PM ET (or 10:30 AM – 7:00 PM ET for MST/PST candidates)
Responsibilities
Pre-Visit Prep
- Preparing members to feel comfortable and at ease with video onboarding prior to their telehealth visit with a Devoted Medical provider (if necessary)
Post-Visit Prep
- Working with home health partners to coordinate DME and skilled nursing services
- Coordinating lab testing and imaging for members, tracking results, and ensuring they are reviewed by the ordering clinician when applicable
- Notifying members of lab results and helping coordinate follow-up care as directed by the clinician
- Coordinating with DM providers on medical record collection, information gathering, appointment scheduling, and outbound communications from Devoted Medical (note: this role does not include clinical decision-making)
Patient Education
- Serving as an advocate for members who call Devoted Medical for care coordination, answering and returning phone calls
- Reviewing internal program referrals, sending welcome kits and other educational materials
Required skills and experience
- MA Certification (highly preferred; expired certifications OK). Verification of active or expired certification required.
- Two (2) years of patient-facing experience, with telehealth experience considered a bonus
- Prior primary care experience
Desired skills and experience
- Experience in care coordination, especially helping Medicare patients close care gaps and complete preventive screenings (e.g., A1c, blood pressure, colorectal cancer screenings, diabetes eye exams, breast cancer screenings)
- Experience in settings such as palliative/hospice, oncology, dialysis, behavioral health, CHF, or diabetes
- Highly organized, adaptable, able to manage a large member panel (100+ if needed) amid frequent changes
- Thrives in a fast-paced, high-energy, rapidly growing organization
- Skilled at easing stress and anxiety for members during difficult times
- Strong communicator with ability to interact effectively with doctors, pharmacies, nurses, and caregivers
- Comfortable working with members via video, phone, or portal
- Entrepreneurial self-starter who takes initiative and thrives as a collaborative team player
- Quick to adopt new technologies, including electronic health records and various computer platforms (e.g., Google Chrome, Google Sheets)

Salary Range: $23.00 - $25.00 per hour
