Qualifications

  • Licensed in NYS as Social Worker or Mental Health Counselor (LCSW/LMHC).
  • Experience with Serious Mental Illness (SMI) required.
  • Supervisory experience required.
  • Experience on ACT teams, inpatient or outpatient psychiatric settings preferred.
  • Experience with criminal justice systems preferred.
  • Strong commitment to rights and autonomy of people with mental illness.
  • Ability to work with diverse individuals and build trusting relationships.
  • Valid NYS driver's license; own vehicle required for business use.

Job description

Clinical Assistant Program Director

The Clinical Assistant Program Director will work closely with the Clinical Program Director to oversee and support the operation and development of the program ensuring services are of the highest quality, and in compliance with policies and procedures. In addition, this position is a clinical staff member of the Forensic Assertive Community Treatment Team (FACT) which is a multidisciplinary treatment team serving adults who have a diagnosis of mental illness and who are justice involved. Responsibilities include providing a full range of services that are culturally relevant and recovery based. These services may occur in the home, community, and office. Services provision is designed to promote recovery by improving psychiatric symptoms, preventing relapses, teaching skills, providing direct assistance, and securing necessary community resources.

  • Assist the Clinical Program Director with the overall operation and development of the FACT Team, including program performance, quality assurance, regulatory compliance, staff productivity, and commitment to the FACT model. Assist with the Assisted Outpatient Treatment (AOT) process and related coordination, monitoring, documentation, and communication.
  • Assume supervision of assigned team members. Assist with staff training and growth to bolster professionalism and expertise. Ensure that designated staff members deliver and document services in a manner consistent with agency procedures and all related policies to State and Federal regulations including the NYS Office of Mental Health (OMH), and OMIG/Medicaid.
  • Conduct person centered comprehensive assessments, including but not limited to, psychiatric history, family/social influences, physical health, substance use, educational/vocational goals, legal status, trauma history, housing stability, entitlements, individual preferences, and life goals.
  • Engage individuals we serve in a respectful, strength-based, and culturally competent manner. Assess individuals’ needs and strengths for independent living skills. Develop and implement strength-based individual treatment plans that are responsive to individuals we serve needs and facilitate achievement of individual goals.
  • Provide support, treatment, medication supervision, case management, community advocacy, and linkage services, per the treatment plan. Complete required documentation of interventions as well as individual's responses and outcomes.
  • Collaborate with all providers of the individual’s care team to ensure the best possible outcome.
  • Readily respond to all crisis situations and urgent needs using sound clinical judgment in order to ensure the safety and well-being of the individual. Provide on-call crisis intervention services on a rotating schedule amongst team members. Always act to ensure individual’s safety, readily assesses needs, provide support and interventions as needed and secure crisis interventions in urgent situations.
  • Mohawk's programs and offices are multi-level with stairs only. All employees must be able to travel to all levels of the facility in a safe and reasonable time frame. Services in the community include visits to individuals’ apartments which can be on the second and third floors.
  • Provide transportation to individuals we serve when necessary, using agency vehicles. Employees must have access and be willing to use personal vehicle to attend individuals we serve meetings in the community and meetings outside the office/program.
Skills/Qualifications for the Clinical Assistant Program Director:

1. NYS Licensed Clinical Social Worker or Licensed Mental Health Counselor.

2. Experience working with individuals with Serious Mental Illness (SMI) required.

3. Supervisory experience required.

4. Experience working on an ACT Team, inpatient, and/or outpatient psychiatric setting preferred.

5. Experience with the criminal justice systems preferred.

6. Must have strong commitment to the rights and ability of each person with a mental illness to make their own decisions and choices, live in the community, work, and have access to continuous support and services to enhance wellness and recovery.

7. Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.

8. Valid and insurable NYS driver's license. Must have your own vehicle to use for business purposes. Agency vehicles are available for transportation of the individuals we serve.

Salary and Schedule of the Clinical Assistant Program Director:
  • $79,164.80/annual equivalent; $3,044.80/biweekly.
  • Full-time, 40 hours per week. Monday-Friday, 8:30-4:30. Rotating after hours on-call, $210 on-call stipend per week.
