Responsibilities

• Coordinate with patient outreach and social determinants of health resources to assist patients.
• Identify high-risk patients for targeted care.
• Conduct post-discharge follow-up calls using Transitional care management protocols.
• Assist patients as they move between care settings to prevent complications and readmissions.
• Reconcile discharge medication lists and support care transitions.
• Collaborate with an interprofessional team in care coordination.
