Job summary

Acentra Health is seeking a Clinical Assessor, Mid to join our team to perform needs-based eligibility determinations for Medicaid-funded home and community-based services. This role requires travel and may involve lifting up to 50 pounds, with responsibilities across assessments and care planning.

The position emphasizes privacy, accuracy, and timely coordination with beneficiaries, families, and providers, including participation in mediation, appeals, and service plan reviews.

Qualifications

• Registered Nurse license in the applicable state.
• Minimum of two years' experience in a home care setting preferred.
• Valid driver's license and willingness to travel to various locations.

Responsibilities

• Conduct assessments to determine eligibility for Medicaid-funded home- and community-based services.
• Assess level of care to support home and community-based settings.
• Maintain privacy and dignity of beneficiaries per HIPAA and policy.
• Coordinate with families, caregivers, and ADW providers as needed.
• Complete assessments and participate in review and appeals processes.
