Job summary

CommonSpirit Health is seeking a Clinical Ambulatory Pharmacist to join the ambulatory specialty care team, partnering with physicians, APPs, and clinic staff to optimize medication therapy for patients with complex chronic diseases and to support the growth of specialty pharmacy services.

Qualifications

• Doctor of Pharmacy degree.
• Clinical experience with 3 years of direct patient care in ambulatory care setting.
• Nebraska Pharmacist License.

Responsibilities

• Partner with physicians, advanced practice providers, and clinic staff to optimize medication therapy.
• Coordinate specialty medication management activities including medication access and prior authorization support.
• Monitor laboratory results, medication safety, and treatment response.
• Assess immunization needs and provide patient education.
• Conduct ongoing clinical follow-up with patients in collaboration with the care team.
• Support timely therapy initiation with patients and interdisciplinary teams.
• Help improve medication adherence and resolve medication-related problems.
• Serve as a clinical resource for providers and staff.
• Contribute to quality improvement initiatives.
• Help develop workflows and implement best practices for the specialty pharmacy program.

Job Summary And Responsibilities

As our Clinical Ambulatory Pharmacist, you will serve as an integral member of the ambulatory specialty care team, partnering with physicians, advanced practice providers, and clinic staff to optimize medication therapy for patients with complex chronic diseases. You will also help support the development and growth of specialty pharmacy services while delivering safe, evidence-based, patient-centered care.

Every day you will work closely with patients, specialty pharmacies, and interdisciplinary healthcare professionals to facilitate timely therapy initiation, improve medication adherence, monitor treatment response, and identify and resolve medication-related problems. You will coordinate specialty medication management activities including medication access, prior authorization support, laboratory and safety monitoring, immunization assessment, patient education, and ongoing clinical follow-up in collaboration with the care team.

Pay Range
