Job summary

Qualifications

• Bachelors with CASAC license, or Masters in Social Work, Mental Health Counselor, or MFT from an accredited school.
• Must have one of: CASAC, MSW, MHC, or MFT license.
• At least one year of experience in addiction counseling.
• OHS Annual Assessment Required.
• OHS Fit Test Required.
• Computer skills including Outlook and Microsoft Word.

Responsibilities

• Provide individual and group counseling and case management to patients in integrated substance abuse and primary care.
• Develop, implement, and review treatment plans quarterly, semi-annually, and annually.
• Coordinate services within the clinic and manage referrals and documentation in charts.
• Monitor methadone treatment and conduct toxicology reviews as part of ongoing care.
• Enter patient data into HRA/STARS database and maintain records as required.

Education

Bachelor’s with CASAC license, or Master’s in Social Work, Mental Health Counselor, or Marriage and Family Therapist from an accredited school
