Position Overview:
Under the supervision of the Clinical Service Director and Bluestem PACE Medical Director, the RN/LPN collaborates with other PACE providers. Responsibilities include assessing, coordinating, monitoring, and providing healthcare services and treatment for Bluestem PACE participants, as well as supporting quality improvement initiatives, data collection, and analysis.
Essential Job Functions:
- Conduct initial comprehensive assessments and participate in IDT.
- Facilitate the integration of new participants into the PACE system, including medication management, immunizations, and monitoring of chronic conditions.
- Provide excellent customer service to participants, families, and colleagues.
- Perform ongoing nursing assessments and document in compliance with data requirements.
- Monitor and deliver routine nursing care, documenting progress and care plans.
- Coordinate with primary care providers and assist in managing episodic illnesses.
- Follow nursing standards and adhere to policies and procedures.
- Administer and monitor medications as prescribed.
- Provide health education and counseling to participants and caregivers.
- Ensure timely and accurate documentation of nursing services.
- Maintain confidentiality of participant information.
- Provide basic first aid to staff as needed.
- Participate in case management and interdisciplinary team activities.
- Ensure participant safety, identify hazards, and respond to emergencies.
- Attend staff meetings, trainings, and committees.
- Comply with the Participant’s Bill of Rights and all regulations.
- Report changes in health status and coordinate admissions, discharges, and transfers.
- Participate in quality assurance activities.
- Prepare and administer medications and treatments as ordered.
- Support infection control and safety protocols.
- Perform other duties as assigned and maintain a professional appearance.
- Be flexible with schedule and on-call duties; may require use of personal vehicle.
- The RN/LPN may perform duties of CNA, CMA, or other nursing staff when needed.
Qualifications and Experience:
- Must be at least 18 years old.
- Active Kansas RN or LPN license required.
- Diploma from an accredited nursing program.
- Proficiency with Microsoft Office Suite.
Physical Requirements:
- Medium work: exerting 20-50 pounds of force occasionally, 10-25 pounds frequently, and up to 10 pounds constantly.
