Apply the nursing process to assess, plan, implement, and evaluate care and teaching needs through direct contact, telephonic/telehealth, or electronic encounters. Collaborate with the healthcare team to coordinate care and maintain compliance with professional nursing standards and regulatory requirements.

Responsibilities

• Assess patients’ physical, emotional, and spiritual needs and develop individualized care plans. • Implement nursing interventions and provide patient education via in‑person, telehealth, or electronic modalities. • Coordinate with multidisciplinary team members to ensure seamless continuity of care. • Maintain accurate and timely documentation, adhering to professional and regulatory guidelines. • Represent Providence’s core values in all patient interactions.

Required Qualifications

• Graduate of an accredited registered nursing school.
• Associate’s Degree in Nursing or Bachelor’s Degree in Nursing.

Preferred Qualifications

• Advanced Cardiovascular Life Support (ACLS) certification upon hire.
