Qualifications

Responsibilities

  • Deliver safe, high-quality patient assessments and follow-up care in ambulatory setting.
  • Perform OB intake, medical assessments, and chronic disease management.
  • Provide patient education on health, medications, and lifestyle.
  • Triages patients by phone and in-person and responds to urgent situations.
  • Coordinate care with providers and care team; participate in planning and discharge.
  • Oversee medication management and immunizations per policy.
  • Assist with in-house labs and data entry in EHR/EPM systems.

The OB/GYN & Women’s Health Nurse is responsible for providing nursing care and assisting healthcare providers in an ambulatory care setting. This position focuses on providing continuing health care to patients, families, and the community while functioning as an integral member of the patient care team.

Requirements
Duties & Responsibilities:
  • Works in partnership with providers and other clinical support staff to deliver safe, high-quality patient assessment, treatment, and follow-up care in a patient-centered ambulatory setting.
  • Assesses patients, recognizes abnormalities or urgent concerns, and seeks appropriate assistance.
  • Performs patient assessments and care planning, including OB intake, medical assessments, chronic disease management, and self-management goal setting.
  • Provides patient education related to health and disease management, including diet, exercise, medications, and other identified teaching needs.
  • Performs patient triage, including telephone triage and walk-in patients, and responds appropriately to urgent or emergent situations.
  • Oversees medication management, including administration of immunizations and other medications and processing medication refills according to InterCare policies and procedures.
  • Performs in-house laboratory testing in accordance with InterCare policies and procedures.
  • Collaborates with providers and other care team members to connect patients and families with appropriate community resources.
  • Participates in pre-visit planning, screening, assessment, discharge, and patient follow-up.
  • Performs general care management and population health duties as needed.
  • Helps oversee clinic flow and coordinates care among members of the clinical support team.
  • Participates in daily huddles with providers, clinical support staff, and other members of the patient care team.
  • Identifies patient flow concerns and takes initiative in developing solutions.
  • Participates in practice improvement initiatives, training, education sessions, and assigned meetings.
  • Supports the competency and clinical development of other team members.

Required Education and Experience:
  • Graduate of an accredited nursing program.
  • CurrentRegistered Nurse (RN) license in the State of Michigan.
  • A minimum of one year of ambulatory and/or hospital nursing experience is preferred.
  • BLS certification is required and must be completed within 90 days of hire.
  • Knowledge of prevailing standards of care, clinical guidelines, and patient care policies and procedures.
  • Ability to meet applicable safety and regulatory requirements, including Joint Commission National Patient Safety Goals, Infection Control, OSHA, and annual competencies.
  • Proficiency with electronic health record (EHR) and electronic practice management (EPM) systems.
  • Ability to effectively utilize email and other required organizational computer systems and applications.

Job duties are performed in a typical office and clinical environment with the required ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs. This position requires good hand-eye coordination, reaching with arms and hands, and finger dexterity, including the ability to grasp and manipulate when using a keyboard and various office and clinical equipment. Visual acuity is required to use a computer and keyboard, operate office equipment, and read printed material. The position also requires the ability to hear and communicate clearly with patients and staff both by telephone and in person.
