Job summary

Community Health Systems is seeking an experienced Licensed Practical Nurse (LPN) for a clinic setting in Fort Wayne, Indiana. The LPN will provide direct patient care under supervision, administer medications, assist with procedures, and document patient information in the EMR.

The role emphasizes patient education, care coordination, and adherence to HIPAA and safety protocols. Typical hours are Monday to Friday, 8:00 AM to 5:00 PM, with no weekend or holiday shifts or on-call requirements.

Qualifications

• Proficiency in direct patient care and medication administration.

Responsibilities

• Perform patient intake, record vital signs and medical history in EMR.
• Administer medications via oral, inhalation, or injection routes and assist with procedures.
• Conduct EKGs and basic lab tests.
• Communicate with patients via portal, phone, or messages and ensure timely follow-up.
• Educate patients on treatment plans and preventive care.
• Collaborate with providers to support care plans and patient needs.
• Maintain cleanliness and proper disposal of hazardous materials in clinic.
