Responsibilities

This position involves providing activities and services related to care for patients with specific diagnoses in subspecialties. Responsibilities include screening inpatient, emergency, and referral lists to identify patients for enrollment, scheduling tests, coordinating referrals, overseeing clinic activities, coordinating patient care, providing education, developing resources, maintaining relationships with physicians, and developing a program database.

Patient Referral

• Facilitate referrals to the sub-specialty clinic, coordinate testing, and collate results using appropriate software.

Patient Care Coordination

• Serve as a resource for diagnosis identification, tracking, and treatment options.
• Act as the primary contact for patients, facilitating communication among patients, families, and providers.
• Develop and implement care plans collaboratively.
• Monitor patient progress and adjust care as needed.
• Create tracking systems and facilitate services.

Education and Support

• Educate staff, patients, and families about diagnoses, medication adherence, and lifestyle modifications.
• Offer emotional support and coordinate educational activities.

Case Management

• Assess needs, coordinate services, schedule follow-ups, and ensure proper documentation.

Quality Improvement

• Participate in initiatives, analyze data, and develop best practices.

Resource Management

• Connect patients with community resources, assist with insurance navigation, maintain databases, and perform other duties as assigned.

Qualifications

• Valid RN license from the state licensing board.
• Basic Life Support (BLS) certification.
• 3-5 years of Cardiac, Thoracic, or Critical Care Nursing experience.

Pay Range

$33.52 - $48.60 /hour
