Job summary

The Licensed Practical Nurse (LPN) works under the direction of a Registered Nurse (RN) and reports to the primary care nurse manager or primary care assistant nurse manager. The LPN is responsible for observing, recording, and reporting changes in the patient's condition through in person or virtual care, and assists in developing and revising the patient-driven holistic care plan for life.

The LPN is a member of the patient healthcare Teamlet.

Qualifications

• Licensed practical or vocational nursing education and licensure as required.
• English language proficiency for direct patient care positions.
• Citizenship requirement for United States employment.

Responsibilities

• Makes observations and thoroughly documents care and the patient's response.
• Performs nursing care in Primary Care Clinics for both in person and virtual care modalities.
• Prescribes or administers medications as permitted and documents actions.
• Provides education to patients and families and assists in orientation of staff and students.

Education

Graduate of a practical or vocational nursing program

Licensed practical nursing licensure
