Job summary

The Licensed Practical Nurse (LPN) provides outpatient ambulatory and home nursing services to eligible individuals under the supervision of the Clinic Manager or Physician Supervisor. Focus is on continuity of care, patient education, and preventive health maintenance.

The role covers direct patient care, policy support, and coordination with the medical team to arrange appropriate care while maintaining safety and confidentiality in a tribal health setting.

Qualifications

• Education: High School diploma or equivalent.
• Graduated from a Practical Nursing Program.
• LPN Licensure in Michigan required.
• Background check and pre-employment drug testing.
• Knowledge of HIPAA regulations and health care laws.
• Knowledge of infection control and safety regulations.

Responsibilities

• Provides and documents direct patient care within the assigned workload.
• Assists in writing and implementing policies and procedures for Nursing and Health Division.
• Ensures the safety and well-being of clients.
• Prepares patient and equipment for appointment and assists provider as needed.
• Identifies patients who require attention and coordinates with the medical team for appropriate care.
• Operates, monitors, and maintains medical equipment.
• Administers PO, IM, SQ, ID, IV medications, and immunizations as ordered.
• Monitors and orders medical supplies and equipment.
• Plans nursing actions based on assessments and best practices.
• Covers staff at other clinic locations as assigned.
• Promotes continuity of care and standardized patient education.
• Schedules work assignments for Nurses’ Aides as assigned.
• Collaborates with health care providers within and outside Health Division.
• Completes home visits as indicated under program or grant requirements.
