Dialysis Clinic, Inc. is seeking a Home Therapies Nurse to coordinate the administrative and clinical aspects of our home dialysis program within the clinic, supporting both home hemodialysis and peritoneal dialysis.

The role focuses on clinical coordination, patient education, interdisciplinary collaboration, and empowering patients to manage their care at home. This is a full-time position, with Monday–Friday shifts starting at 7am and on-call as needed; compensation ranges from $37 to $47 per

Responsibilities

• Collaborate with medical team on patient care concerns and changes.
• Coordinate home training sessions, clinic visits, and education.
• Monitor patient supply ordering and usage while maintaining quality care.
• Communicate regularly with interdisciplinary care team about home program activities.
• Monitor ongoing patient care parameters.
• Perform all registered nurse functions and maintain records per ESRD networks and DCI policies.
