Job Summary

The Licensed Clinical Social Worker 3 (CSW 3) is the lead medical social worker at the UCSF Women’s HIV Program. This role includes clinical and leadership tasks. The CSW 3 provides psychosocial assessment, crisis intervention, brief therapeutic interventions, group facilitation, consultation and clinical supervision, education, community linkages and supportive services for identified patients, families, and staff. Additionally, the CSW 3 assigns new patients to provider and social worker panels, reviews intake assessments, coordinates psychiatric visits, tracks and addresses no-shows and patients who are not following up with their care team and develops team policies and procedures. The CSW 3 demonstrates cultural sensitivity in working with patients regardless of race, gender, gender identity, religious affiliation, cultural beliefs, sexual orientation, etc. The CSW 3 demonstrates ability to work with medically and psychiatrically complex patients in a trauma-informed manner.

Social Worker Functions Specific to the Program

1. Reviewing incoming referral documentation for completeness and appropriateness for the program.
2. Welcoming new patients to the program by meeting with them either in person or by telephone, conducting a psychosocial assessment or assigning psychosocial assessment to another social worker, assigning new patients to a complex care manager. Reviewing psychosocial assessment and treatments plans of new patients.
3. Reviews psychiatry referrals and assigns cases to psychiatric providers, schedules psychiatric appointments, and works with patients to complete psychiatric screenings.
4. Making appropriate referrals to internal behavioral health services and external resources (e.g., housing, legal aid, disability benefits, financial assistance, etc.) based on the patient’s needs.
5. Providing medical case management services (e.g., processing referrals for Home Health support and Durable Medical Equipment, accompanying patients to ER and/or other specialty clinic appointments, etc.) using a model of complex care management.
6. Coordinating and supervising MSW interns.
7. Providing clinical supervision to social work associates and social workers as needed.
8. Coordinating education activities for the WHP social work team.
9. Facilitation of psychosocial and multidisciplinary team meetings.

Required Qualifications

• Graduate with a MSW, from an Accredited School of Social Work.
• Licensed Clinical Social Worker able to Work in the state of California.
• Three (3) years of experience in a behavioral or medical healthcare setting required.
• Two (2) years of experience working with people with HIV.
• Demonstrated experience of practicing cultural sensitivity and humility in working with Black women living with HIV and/or other marginalized populations.
• Familiarity with an electronic medical record program and/or Microsoft Office, Excel, Word and Outlook.
• Knowledge and abilities essential to the successful performance of the duties assigned to the position.
• Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
• Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high-volume work environment.
• Ability to work effectively and collaboratively with interdisciplinary teams.
• Demonstrates case management skills and experience in the community health care delivery system.
• Exceptional communication and presentation skills relating to medically and psychiatrically complex adults, their support systems and teams of health professionals.

Preferred Qualifications

• Bilingual fluency in English and Spanish highly preferred.
• Lived experience of and/or professional experience addressing HIV, trauma, racism and/or other forms of discrimination, substance use, mental health concerns, and chronic health conditions.
• Experience with concepts of Harm Reduction, Motivational Interviewing.

License/Certification

Licensed Clinical Social Worker (LCSW) in the State of California.
