Job summary

Family Options Inc. is seeking a compassionate Client Support Coordinator (CSC) to aid individuals with developmental and behavioral challenges in building independence and community participation.

You will provide direct care, guidance, supervision, transportation, and support while helping clients develop life skills. The role includes administering medications within training, coordinating with teams, and helping clients achieve IPOS/IEP goals in agency programs and community settings.

Qualifications

  • Valid CT driver’s license with satisfactory driving record.
  • Successful completion of Medication Certification Program.
  • Successful completion of PMT, CPR and First Aid training.

Family Options Inc. is looking for a compassionate, dependable, and motivated Client Support Coordinator (CSC) to join our team. This is an opportunity to do meaningful work while helping individuals with developmental and behavioral challenges build independence, reach personal goals, participate in their communities, and live fulfilling lives.

As a CSC, you will be an important part of our clients’ daily lives. You will provide direct care, guidance, supervision, teaching, transportation, and support while helping individuals develop and maintain important life skills.

Starting Rate: $18.75 per hour

Med Certified Rate: $20.00 per hour

PLUS: 16 hours of PTO upon successful completion of the Medication Certification Program for the first time

Full-Time: 40 hours per week

  • Monday: 2:00 PM – 10:00 PM
  • Tuesday: 2:00 PM – 11:00 PM
  • Thursday: 2:00 PM – 10:00 PM
What You’ll Do

The Client Support Coordinator provides habilitative assistance and direct care to individuals living in Agency programs, including community and supported living arrangements, family support services, and day/vocational programs.

You will help clients with daily activities, personal goals, community involvement, health and safety, and developing greater independence.

Your responsibilities will include:

  • Providing direct supervision and knowing the status and location of clients in your care at all times.
  • Supporting clients in achieving goals identified in their Individualized Plan of Service (IPOS) or Individual Educational Plan (IEP).
  • Teaching and assisting with activities of daily living, including hygiene, personal care, eating, shopping, banking, meal planning, clothing care, community participation, and self-control.
  • Monitoring and documenting client progress toward individual goals.
  • Protecting client rights and immediately reporting suspected abuse, neglect, or mistreatment.
  • Completing required DDS and Agency incident reports and medication error reports accurately and promptly.
  • Implementing behavioral support plans as written, including approved emergency procedures and restraints when necessary and properly trained.
  • Following recommendations and programs provided by occupational, physical, and speech therapists, including positioning, sensory stimulation, lifts, and transfers.
  • Planning and participating in recreational and community activities.
  • Working collaboratively with Program Directors, supervisors, clinicians, consultants, families, schools, DDS, and other community agencies.
  • Maintaining accurate and timely documentation of services provided and progress toward individual goals.
  • Administering medications and providing routine health care and first aid within the scope of required training and certification.
  • Transporting clients safely to and from designated activities and programs.
  • Keeping program vehicles, residences, equipment, and common areas clean, safe, and organized.
  • Maintaining accurate records for petty cash and individual client funds.
  • Participating in emergency drills and following Agency and DDS safety procedures.
  • Attending required staff meetings, in-services, and trainings.
  • Maintaining all required certifications and trainings.
  • Performing other related duties as assigned by the Program Director or Assistant.

Required:

  • High School Diploma or equivalent.
  • Valid Connecticut driver's license with a satisfactory driving record.
  • Successful completion of required Medication Certification Program.
  • Successful completion of PMT, CPR, and First Aid training programs.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Ability to follow written and verbal instructions and use sound judgment when addressing day‑to‑day situations.

Preferred:

  • At least two years of experience working with individuals with developmental and/or behavioral challenges, or an equivalent combination of education and experience.
Physical Requirements

This position involves active, hands‑on support. Employees regularly stand, walk, sit, talk, hear, use their hands, reach, and assist clients with daily activities.

The position frequently requires lifting and/or moving up to 75 pounds and may require exerting more than 100 pounds of force when initiating physical restraint procedures. Appropriate training is provided.

The work environment is generally moderate in noise level. The position requires travel by automobile and may involve exposure to changing weather conditions.

Requirements
COMPETENCIES:

Successful completion of Medication Certification Program

Successful completion of PMT, CPR and First Aide training program

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

EDUCATION AND/OR EXPERIENCE

High School Diploma or equivalent. A minimum of two years experience working with individuals with developmental and/or behaviors challenges is preferred; or an equivalent combination of education and experience.

LANGUAGE SKILLS:

Ability to read and interpret English documents such as safety rules, maintenance instructions, medical reports and procedure manuals. Ability to write routine reports and correspondence in English.

MATHEMATICAL SKILLS:

Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.

REASONING ABILITY:

Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving several concrete variables.
