Role

As a Client Relations Support Associate, you will play a key role in building and maintaining strong relationships with healthcare professionals, patients, and caregivers. You’ll be the bridge between our services and those who need them, ensuring seamless transitions, addressing inquiries, and identifying opportunities for growth. Your work will directly impact patient care by connecting them with the resources they need to thrive at home. You will:

  • Build and nurture strong relationships with referral sources, including long-term care facilities, discharge planners, physicians, and social workers.
  • Educate patients, caregivers, and healthcare professionals about our services, including Medicare, Medicaid, and insurance guidelines.
  • Act as a trusted resource, answering questions and addressing concerns in a timely and professional manner.
  • Maintain detailed records on referral sources to optimize communication and service delivery.
  • Represent our organization at professional and community events to increase awareness and foster partnerships.
  • Facilitate the transition of patients from hospitals or care facilities to home health or hospice services.
  • Assist with collecting and delivering essential documents, including physician-signed orders.
  • Track and report field activity results on a weekly basis to ensure alignment with business goals.
  • Identify trends and opportunities to improve services and enhance client satisfaction.
  • Support organizational values by upholding the highest standards of customer service, integrity, and respect.

Qualifications

*Must Reside In One of These States: AR/ IA/ KY/ NE/ TN/ TX/ WA!*

  • High school diploma or equivalent required.
  • Remote in WA State Only.
  • At least one year of experience in healthcare, customer service, or a related field (sales experience a plus!).
  • Two years of payroll related experience preferred.
  • Prior recruiting experience is preferred.
  • Excellent verbal and written communication skills.
  • Ability to manage multiple priorities and work efficiently in a fast-paced environment.
