Base pay range

$60,000.00/yr - $65,000.00/yr

Primary Function/Purpose

The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The role involves assessing clients’ long-term goals at intake, collaborating with case management staff to set achievable goals, and establishing community linkages to support clients' progress towards independent living. This position is based at our Kenilworth location in Brooklyn, NY.

Minimum Qualifications
  • Must be a Licensed Master Social Worker (LMSW) in New York upon hire or obtain licensure within three months.
  • At least one year of experience in social services or related fields is preferred.
  • Knowledge of working with diverse cultures and ethnicities.
  • Experience with strengths-based and solution-focused practices.
  • Experience facilitating groups for adults.
  • Proficiency in Spanish and/or French is a plus.
  • Excellent communication skills.
  • Interpersonal skills and computer literacy in Microsoft Office are required.
Primary Responsibilities
  • Conduct comprehensive bio-psychosocial assessments.
  • Provide clear written reports of assessments and recommendations.
  • Assist homeless clients in navigating systems and managing stressors.
  • Provide outreach, crisis intervention, risk assessment, safety planning, and psycho-education.
  • Prioritize and conduct visits to high-risk individuals in shelter for safety planning.
  • Prepare clients to accept services and facilitate referrals.
  • Collaborate with agencies and participate in conferences for client advocacy.
  • Facilitate access to mental health services via in-person or telehealth options.
  • Coordinate clinical treatment and follow-up services.
  • Promote telehealth usage and assist with scheduling and transportation.
  • Follow up on referrals and re-engage clients as needed.
  • Provide therapeutic groups and participate in team meetings.
  • Assist with housing applications and support post-housing linkage.
  • Complete documentation and reports timely.
  • Participate in professional development and trainings.
  • Perform other duties as assigned.

  • Interact with clients to assess needs.
  • Work with staff to ensure high-quality service delivery.
  • Network with community and government agencies for resources.
