Base pay range

$56,000.00/yr - $60,000.00/yr

Primary Function/Purpose

The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The role involves assessing clients’ long-term goals, collaborating with case management staff, and establishing community linkages to promote independent living. The position is based at Icahn House in the Bronx, NY.

Minimum Qualifications
  • Must be a Licensed Master Social Worker (LMSW) in New York or obtain licensure within three months of hire, with a master’s degree in Social Work from an accredited school.
  • At least one year of experience in social services or related fields is preferred.
  • Knowledge of working with diverse cultures and ethnicities.
  • Experience with strengths-based, solution-focused, and family-centered practices.
  • Experience facilitating groups for adults.
  • Proficiency in Spanish and/or French is a plus.
  • Excellent written and verbal communication skills.
  • Interpersonal skills and computer literacy in Microsoft Office are required.
Primary Responsibilities
  • Conduct comprehensive bio-psychosocial assessments.
  • Provide clear written reports and recommendations.
  • Assist homeless families in navigating systems and coping with stress.
  • Offer outreach, crisis intervention, risk assessment, safety planning, and psycho-education.
  • Prioritize visits to high-risk families in shelter for safety planning.
  • Prepare families for services and collaborate with agencies and service providers.
  • Make referrals and facilitate community linkages.
  • Serve as liaison with government and community organizations.
  • Improve access to mental health services via in-person or telehealth methods.
  • Work as part of a multidisciplinary team to ensure quality service delivery.
  • Deliver on-site mental health services, coordinate treatment, and follow-up.
  • Utilize telehealth and other resources to facilitate access.
  • Follow up on referrals and re-engage clients as needed.
  • Provide therapeutic groups and facilitate group work.
  • Participate in family meetings, assist with housing applications, and support family transitions.
  • Complete documentation and internal/external reports.
  • Attend professional development and training sessions.
  • Perform other duties as assigned.
Principal Relationships
  • Interact with diverse clients to assess needs.
  • Collaborate with staff to ensure high-quality services.
  • Network with community and government agencies for resource support.
Other Requirements
  • Lifting objects up to 30 pounds regularly.
