Qualifications

The Client Care Coordinator is responsible for customer service, client care standards, scheduling of field staff, and ensuring the branch meets or exceeds inquiry conversion goals.

QUALIFICATIONS:

  • Must have completed 75 hour Home Health Aide Certificate Course or CNA License
  • High School Diploma
  • Flexible and strong time management skills
  • Minimum of one-year experience in Private Duty Home Care/Home Health
  • Excellent organization, planning and customer service skills
  • Strong interpersonal and relationship building skills
  • Must understand and respect client’s rights including ethics and confidentiality of care.

RESPONSIBILITIES:

  • Effectively serves as contact for client/customer/caregiver relations.
  • Assists with intake of new clients and informs potential clients of our services. Continued communication and follow up with current clients.
  • Propose services and financial agreements with clients.
  • Accurately inputs new patient referral and contact information into the system and coordinates with RN to schedule assessments and caregivers appropriately.
  • Ensures all scheduling and care coverage by constant communication with field staff, uses technology tools and resources to ensure proper processes; assures staff schedules will meet client care needs of existing and new clients.
  • Ensures schedule coverage with re-staffing call offs, vacation schedules, and all other emergency and non-emergency scheduling issues during shift and on an on-call basis.
  • Accurately entering caregiver and payroll data; confirming timesheet, tasks and visits are complete within the payroll system.
  • Assist with daily duties as needed and assigned by Leadership.
  • Active involvement in hiring and recruitment process. Coordinates with HR the processing of new employee paperwork and setup of employee files.
  • Assisting with caregiver orientation.
  • Assures compliance with all local, state, and federal laws.
  • Understands and adheres to established policies and procedures of agency.
  • Takes rotating on-call 24/7 to screen all calls and forward to the proper staff.
  • Meets with field staff for first visit orientation of all new clients.
  • Maintains a current client roster with necessary information.
  • Maintains an ongoing relationship with current, future and former clients, client families, company sales team, employees and outside agencies.
