Qualifications

Responsibilities

• Provides home-based case management services to pregnant women and their newborns.
• Conducts screening and health assessments for health and social needs and develops comprehensive case management plans.
• Provides prenatal classes and one-on-one counseling on pregnancy, labor, delivery, postpartum, family planning, breastfeeding, newborn care, and child development.
• Works with a multidisciplinary team to reduce barriers to care and health disparities.
• Provides home visiting services within Montgomery County with mileage reimbursed.
• Assists nurses with immunization clinics on a rotating basis.
• Uses Electronic Health Records to document clinical and case management activities.
• Performs other job-related duties as assigned.

WHO WE ARE LOOKING FOR

DHHS Public Health Services Division is currently seeking a compassionate and dedicated Client Assistance Specialist to join our Maternal Child Health team at the Silver Spring Health Center. The candidate must have a desire to serve the community and uphold a commitment to public good. Candidates should display cultural awareness, openness to learning, a positive attitude, a teamwork mindset, and adaptability. Client must have a strong knowledge of Medicaid and have experience guiding clients through the health care delivery system. The Health Center is staffed by Nurses, Community Health Workers, Client Assistance Specialists, and Administrative staff with the primary goal of improving outcomes for high/low risk pregnant women and newborns through home-based case management in Montgomery County (Down County).

Duties of the Client Assistance Specialist include:

• Provides home-based case management services to pregnant women and their newborns.
• Conducts screening and health assessment of pregnant women and newborns for health/social needs and develops comprehensive case management plans to address identified medical, social, and behavioral health needs.
• Provide prenatal classes and one-on-one counseling for clients on topics such as pregnancy, labor and delivery, postpartum, family planning, breastfeeding, newborn care, and child growth and development to optimize outcomes for the women, her newborn, and the family.
• Works with a multidisciplinary team to support pregnant women, their families, and the community to reduce barriers to care and health disparities in the marginalized populations in the County.
• Provides home visiting services within the County in one's own car with mileage reimbursed.
• Assists Nurses with the immunization clinic on a rotating basis.
• Serves as a member of the multidisciplinary team to meet the health and social needs of individuals, families, and the community.
• Utilizes Electronic Health Record to document all clinical and case management activities.
• Performs other job-related duties as assigned.

ADDITIONAL INFORMATION:

Medical Protocol: A medical exam and drug screen will be required for the selected candidate.

Work Location: This position requires the ability to attend meetings or perform work at locations outside the office.

Physical Requirements: This position may require the ability to walk up and down steps when attending meetings or working outside the office.

Multilingual Skills: Candidates with multilingual communication skills (verbal and written) in Spanish, Amharic, or French are strongly preferred.

Education: Graduation from an accredited college or university with a Bachelor’s Degree.

Experience: One (1) year of experience in human services that involved substantive client contact and service delivery through direct assistance or referral. This experience must have included the ability to communicate effectively across a wide range of cultures and overcome potential barriers to service.

Substitutions: An equivalent combination of education and experience may be substituted.
