Essential Duties and Responsibilities


  • Develop and maintain referral relationships with physicians, discharge planners, social workers, nursing facilities, hospitals, and community organizations.

  • Conduct regular in-person visits to referral sources (nursing homes, doctor’s office, clinics, and community organizations) to promote home care services and generate new client referrals.

  • Identify prospective patients and referral opportunities and convert referrals into admissions.

  • Meet established referral, admission, and business development goals.

  • Conduct community outreach and networking activities to increase market awareness and generate new business opportunities.

  • Track referral activity, admissions, and sales pipeline metrics.

  • Attend community events, healthcare networking events, and professional meetings to develop referral relationships.

  • Present and explain home care service offerings to prospective patients, families, and referral sources to facilitate enrollment and admission decisions.

  • Facilitate the admission process for prospective clients and coordinate the transition from referral to enrollment.

  • Advocate for patients to ensure appropriate services and authorizations.

  • Develop and maintain strategic referral relationships that generate ongoing admissions and business growth.

  • Coordinate with internal teams on referrals and authorizations.

  • Maintain accurate documentation and records.

  • Provide updates and reports to leadership.

  • Consistently develop new referral relationships while maintaining existing accounts.
