Base pay range will be determined by applicant’s skills and experience. Role is also eligible for team-based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities

Provides dementia care including adapted ADL’s, meaningful engagement, and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.

  • Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and getting to and from programs and meals according to the individual service plan and interventions. Encourages residents to be as independent as possible.
  • Participates in team behavioral expression meetings or discussions with community leadership.
  • Knows the residents' life stories, preferences, routines, and interests.
  • Serves meals in the dining room. May assist in meal preparation following dementia-friendly menus. Prompts residents to eat properly and stay hydrated, respecting their right to free choice. Reports any changes in eating habits to the supervisor.
  • Initiates and/or leads at least one Daily Path engagement program as per the resident’s plan. Encourages socialization and participation in person-centered programs and supports friendships among residents.
  • Maintains accurate residents' records daily, including documentation related to medication, engagement, incidents, observations, and health changes. Reports any changes promptly.
  • If permitted by state regulations, may assist with medication following physician orders and state laws. Follows medication policies and procedures.
  • Maintains a clean and safe environment, performs housekeeping tasks, and follows cleaning schedules.
  • Responds promptly to emergency situations and resident requests for assistance.
  • Monitors residents' locations and reports if a resident is unaccounted for.
  • Encourages teamwork through cooperative interactions with co-workers.

Qualifications

Education and Experience: High school diploma or GED preferred, with 3-6 months related experience or training in dementia care or equivalent.

Certifications and Licenses: May require current state certification and adherence to regulations to maintain it.

Physical Demands and Conditions: Includes standing, walking, sitting, handling, reaching, climbing, stooping, talking, hearing, tasting, smelling, lifting up to 50 pounds, and exposure to various hazards and infections. May require travel and occasional weekend or evening work.

Decision Making and Skills

Makes routine decisions based on detailed guidelines. Has basic skills to follow instructions, read and write simple correspondence, and communicate effectively in English. Uses good judgment especially in emergencies.
