Description

Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.

1. Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and attending programs and meals according to the individual service plan and interventions. Encourages residents to be as independent as possible, supporting self-care.
2. Participates in team behavioral expression meetings or discussions with community leadership.
3. Knows residents' Life Stories, preferences, routines, and interests.
4. Serves meals in the dining room, assists in preparing meals following dementia-friendly menus, and encourages proper diet and fluid intake, respecting residents' rights to free choice. Reports any changes in eating habits.
5. Initiates or leads at least one Daily Path engagement program, encouraging socialization and participation in person-centered activities.
6. Maintains residents' records daily, including documentation related to medication, interests, incidents, observations, and health changes, in accordance with company policies.
7. Observes and reports residents' health and emotional conditions, following medication policies and state regulations regarding medication assistance.
8. Maintains a clean, safe environment, performs housekeeping tasks, and responds promptly to resident requests and emergencies.
9. Monitors resident locations and reports if a resident cannot be located.
10. Encourages teamwork and cooperative interactions with co-workers.

Applicants' skills and experience determine the base pay within the range. The role may include team-based bonus opportunities. Temporary associates are not benefits-eligible but may participate in the 401(k) plan.
