Job summary

Phamily Staffing is assisting North Carolina Nephrology in hiring a Phamily Care Coordinator to support nephrologists and advanced practitioners in managing chronic care patients. This role uses the Phamily Chronic Care Management platform to improve patient care, prevent unnecessary hospitalizations, and enhance communication between patients and their care teams.

Based at the Wilson Office with a Monday–Friday 8:00 a.m. to 4:00 p.m.

Qualifications

• 1–3 years of healthcare experience with patients who have complex chronic diseases (preferred).
• Strong working knowledge of chronic kidney disease (CKD).
• Excellent organizational, communication, time management, and multitasking abilities.
• Proficiency with reading, writing, and verbal communication; basic EHR skills.

Responsibilities

• Manage initial enrollment and ongoing CCM patient registrations.
• Validate CCM enrollment per NCN guidelines.
• Ensure PHI consent before any health-related communication.
• Maintain a minimum of 20 minutes of Phamily/telephonic communication per patient each month.
• Document interactions in Phamily and the EHR per CCM requirements.
• Monitor adherence to care plans, track progress, and recommend adjustments.
• Coordinate continuity of care across healthcare organizations and facilities.
