A Day In The Life Of This Role

Reporting to the Director of Nursing, the Chronic Pain and Addiction Case Manager provides clinical leadership, care coordination, and nursing support for patients managing chronic pain, opioid‑related conditions, and substance use disorders.

This role combines complex care management for chronic pain, medication‑assisted treatment coordination, patient education, and interdisciplinary collaboration to improve patient safety, treatment adherence, recovery outcomes, and overall quality of life.

Summary Of Duties

Chronic Pain Case Management

• Perform comprehensive nursing assessments for patients with chronic pain, including pain history, functional status, medication use, psychosocial needs, and barriers to care.
• Develop, implement, and monitor individualized care management plans focused on improving function, safety, and patient‑centered outcomes.
• Coordinate care with primary care providers, pain specialists, behavioral health clinicians, physical therapists, pharmacists, and community resources as needed.
• Provide education on chronic pain conditions, non‑opioid pain management strategies, medication safety, coping skills, and self‑management techniques.
• Monitor treatment effectiveness, medication adherence, adverse effects, and changes in patient condition.
• Support safe opioid prescribing practices through patient monitoring, risk assessment, care coordination, and provider communication.
• Assist patients with navigating healthcare systems, referrals, insurance barriers, and access to supportive services.
• Track clinical outcomes, follow‑up needs, and progress toward individualized goals.
• Screen for high‑risk behavior, early recognition and transition to substance use treatment as applicable.

Addiction Case Management

• Provide support to the Addiction Clinical Specialist to assist with management of complex substance use cases and MAT inductions.
• Act as a clinical liaison to the Behavioral Health Recovery Support team.
• Assist in panel monitoring and primary care team support as needed.
• Conduct nursing intake assessments, health screenings, medication reconciliation, and ongoing patient monitoring.
• Monitor patient response to treatment, withdrawal symptoms, cravings, recovery goals, and treatment engagement.
• Provide patient education regarding addiction as a chronic disease, overdose prevention, harm reduction strategies, naloxone use, and recovery support.
• Manage workflows, including patient outreach, follow‑up tracking, care gaps, and referral coordination.
• Coordinate medication ordering/refills/delivery to sites.
• Support quality improvement initiatives related to substance use disorder treatment access, retention, safety, and outcomes.
• Assist with development and implementation of clinical protocols, workflows, and staff education related to addiction treatment.
• Other related duties and projects as assigned.

Qualifications

What we need from you:

• BLS/CPR certification required.
• Minimum of 2+ years of clinical nursing experience required.
• Experience in chronic pain management, addiction medicine, behavioral health, primary care, or complex care management.
• Knowledge of opioid use disorder treatment, medication‑assisted treatment (MAT/MOUD), and harm reduction principles.
• Experience or desire to work in a team-based care model.
• Bachelor of Science or Associates in Nursing from an accredited school of nursing by AACN.
• Current Registered Nurse (RN) license from the Massachusetts State Board of Registration.
• Maintain and enhance professional nursing skill and knowledge by attending continuing education programs.
• Excellent interpersonal skills, written and verbal communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
• Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive, etc.
• Excellent judgment and ability to solve problems in a timely manner.
• Ability to adhere to strict confidentiality standards.
• Demonstrated ability to work effectively both as a team member and independently within a culturally diverse and inclusive environment.
