Job summary

WCHS is seeking a Chronic Disease Prevention, Wellness, and Screening Coordinator to lead community-based chronic disease prevention and management initiatives, including diabetes prevention, cardiovascular health, and nutrition programs.

The role involves coordinating screenings, coaching individuals and groups, and collaborating with healthcare providers and schools to improve population health outcomes in the Winnebago area.

Qualifications

• Minimum of two years of experience in field/public health.
• Experience coordinating community health screenings and wellness programs.
• Experience with diabetes prevention/management and population health.
• Knowledge of motivational interviewing, health coaching, and behavior change techniques.
• Experience working with Indigenous communities and culturally responsive health promotion preferred.
• Knowledge of Diabetes Practice Guidelines and case management techniques.
• Knowledge of principles of diabetes education and culturally appropriate delivery models.
• Valid driver's license; ability to pass background checks.

Responsibilities

• Coordinate chronic disease prevention and management activities targeting diabetes, hypertension, cardiovascular disease, obesity, and other chronic conditions.
• Develop and implement evidence-based programs that support disease prevention, self-management, and healthy behavior change.
• Monitor participant progress and assist with care coordination, referrals, and follow-up services.
• Serve as a Lifestyle Coach for NDPP and other chronic disease prevention initiatives.
• Provide individual and group coaching related to nutrition, physical activity, stress management, sleep health, tobacco cessation, and healthy lifestyle behaviors.
• Facilitate wellness classes, workshops, support groups, and educational events.
• Develop and maintain collaborative relationships with healthcare providers and community organizations.
