Qualifications

• Requires an Associate’s degree (AA) from a two-year college or technical school.
• 1–2 years related experience in underserved populations or community settings; 2–4 years in Case Management or Care Coordination.
• Valid Oregon Driver’s License with vehicle insurance; BLS, CPI, MHFA certifications or willingness to obtain within 90 days; CCM preferred or pursued within 2 years.

Responsibilities

• Perform assessments, planning, and coordinating services to meet members’ health care needs.
• Develop and manage individualized care plans with regular re-evaluation.
• Collaborate with interdisciplinary care teams (ICT) to support chronic disease management.
