The Community Health Representative (CHR) Generalist serves as a liaison between healthcare providers, community resources, and tribal members to improve access to healthcare and supportive services. Under the direction of the CHR Director, the CHR Generalist provides health education, transportation coordination, home visits, medication assistance, case management support, and community outreach while promoting wellness and disease prevention among eligible clients.
The CHR Generalist works closely with healthcare providers, hospitals, Indian Health Service (IHS), tribal departments, and community organizations to help clients overcome barriers to care and improve their overall quality of life.
Works under the direct supervision of the CHR Director and CHR Admin Assistant.
Office space is assigned for documenting, scheduling and client coordination. Most work is out in the field in all weather conditions.
