Job summary

Communicare is seeking a licensed behavioral health clinician to provide evidence-based services in clients' homes or community settings, maintaining fidelity to care plans and ensuring continuity of care.

You will arrange for medication evaluation and monitoring, participate in treatment teams, document promptly in Communicare's EHR, travel throughout the catchment area, and complete required crisis intervention trainings.

Qualifications

• Master’s degree and license or eligibility for licensure in behavioral health fields.
• Ability to work independently and relate well to clients and families.
• Willingness to travel within the catchment area and to trainings.

Responsibilities

• Provide evidence-based mental health services in clients' residences or community settings.
• Arrange for medication evaluation, management, and monitoring.
• Maintain active caseload and meet productivity and documentation standards.
• Work with the treatment team to ensure continuity of care.
• Utilize and update Communicare’s EHR system and participate in trainings.
