Responsibilities

• Manage own caseload of children in the community.
• Provide evidence-based nursing care to children in home settings with assessment, care planning, delivery and evaluation.
• Plan programmes of care to promote health gains and independence in various settings (home, school, etc).
• Make autonomous clinical judgments and liaise with Matron and multidisciplinary colleagues to manage health needs.
• Identify when a child’s condition warrants discharge from the service.
• Develop and evaluate training programmes for families, carers and the multidisciplinary team.
