Job summary

Center for Life Management provides outpatient mental health services for children and families, ages 3–21, in the NH area. The Children’s Community Clinician role involves diagnostic assessments, treatment planning, crisis management, and individual/family/group therapy in home and office settings, with some telehealth options.

This is a full-time position offering 35 or 40 hours per week. We offer a comprehensive benefits package and a $5,000 sign-on bonus paid in installments.

Qualifications

  • Master’s degree required.
  • LISCW, LCMHC or LMFT preferred.
  • Experience providing family therapy and working within a multi-disciplinary team.
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.
  • Ability to actively engage with children and conduct interactive play within a therapeutic setting.
  • Proficient computer skills, specifically with all Microsoft Office applications.
  • Ability to document effectively and efficiently in course of clinical work.
  • Ability to navigate an electronic health record.
  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision.

Responsibilities

  • Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes.
  • Complete assessments for eligibility determination for State funded services.
  • Provide individual, family, and group therapy in the office and/or remote via telehealth.
  • Provide Crisis Intervention when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
  • Provide timely documentation on each service delivered.
  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around family systems approach is provided to the client.
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.

CLM provides the area's most comprehensive array of mental health services for children and their families. Our multi-disciplined treatment team works with individuals ages 3 – 21 who experience mental illness and/or serious emotional difficulties. The Children’s Community Clinician provides outpatient clinical services including diagnostic assessment, treatment planning, crisis management, individual therapy, family therapy, and group therapy in the child/family’s natural environment. This is a full time, 35 or 40 hour per week position.

Major Functional Areas and Description of Duties
  • Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives;
  • Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes;
  • Complete assessments for eligibility determination for State funded services;
  • Provide individual, family, and group therapy in the office and/or remote via telehealth;
  • Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;
  • Provide timely documentation on each service delivered;
  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around family systems approach is provided to the client;
  • Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation);
  • Serve on appropriate committees, both internal and external, as determined by supervisor;
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;
  • Execute all required services and documentation in accordance with agency policies and federal/state standards;
  • Maintain professional memberships, licenses, accreditation, certifications, etc.;
  • Represent CLM in a professional and appropriate manner in all settings;
  • Perform all other duties as assigned by supervisor.
Physical Demands and Working Conditions

A majority of the work will be visiting clients in the community and also in their homes. Some remote and/or telehealth work may be required at times. Manual dexterity is needed to use computers and standard office equipment. The ability to sit or stand for extended periods of time is required. Reliable internet, phone, transportation, a clean driving record, and proof of auto insurance coverage are required.
