Job summary

AMHC is seeking a compassionate Children’s Community Case Manager to join our Calais team. This full-time role focuses on coordinating care for children with behavioral health needs, working with families, providers, and community partners to implement treatment plans and advocate for services.

You will lead care planning, collaborate with multidisciplinary teams, and ensure documentation and compliance while supporting families through crises and ongoing care navigation.

Qualifications

• Bachelor’s degree required; field options include psychology, mental health, human services or related.
• Must complete CANS assessment tool and pass annually.
• Must have valid driver’s license and reliable transportation.

In this key role, you’ll coordinate care and support services for children with behavioral health needs, working closely with families, treatment providers, and community partners. Schedule: This is a non-exempt full-time position (40 hours/week).

Requirements

• The position requires a bachelor’s degree with a specialization in psychology, mental health, human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field. OR have a bachelor’s degree from an accredited four (4) year educational institution in an unrelated field and at least one year of full-time equivalent relevant human services experience.


• Must complete the Child and Adolescent Needs and Strengths (CANS) assessment tool and pass the exam annually.


• Must possess a valid driver’s license and reliable transportation.
