Job summary

Pathways, Inc. is seeking a Children's Care Manager in Corning, NY to provide care management services to children and adults enrolled in Regional Health Homes, following established care protocols.

Applicants should hold a Bachelor's degree in Social Work, Psychology, or related Human Service field, have 1-2 years of direct services or care coordination experience, possess a valid driver's license, and be able to pass required background checks.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related Human Service field.
• Valid driver’s license with insurability.
• Must clear State Central Registry, Staff Exclusion List, and DOH Criminal History checks.

• Provide care management services to children and/or adults enrolled in Regional Health Homes.
• Follow established protocols for delivery of care management.

Nature of Work:
The Care Manager is responsible for the provision of care management services to children and/or adults enrolled in the Regional Health Homes. All work will be carried out in accordance with well-established protocols for the delivery of care management services.

• Based on education level, a minimum of a 1-2 years of relevant experience is required in direct services, linkage to services, care coordination, health promotion, transitional care, and/or individual and family support. Waiver may be available based on experience.
• It is required that candidates have a Bachelor’s degree in Social Work, Psychology, or a related Human Service field. Waiver may be available based on experience.
• A valid driver’s license with driving record that meets insurability guidelines to operate agency-owned vehicles.
• Must clear the State Central Registry Clearance, Staff Exclusion List Clearance and DOH Criminal History Review for criminal convictions.
