Qualifications

• Valid Tennessee Driver’s License required.
• Acceptable Motor Vehicle Report (MVR).
• Acceptable Criminal Background Investigation required.
• Personal Automobile Insurance mandatory.

Responsibilities

• Engage with consumers monthly to assess needs and coordinate care.
• Complete visits with consumers in the community.
• Address mental health and physical health needs.
• Assist with referral and linkage to community resources.
• Participate in daily team meetings and share insights.
