Complete child development screening tools with patients during visits or over the phone. Explain the importance of monitoring child’s development and how screenings will be used to evaluate a child’s developmental progress and identify any concerns. Ensure screening tools are appropriately documented in the patient’s electronic health record. Work collaboratively with pediatric care team to identify missing care gaps (such as immunizations) and Parent Educator/Case Manager to support patients and families with needed referrals.
