If you are passionate about making a difference in your community, we invite you to consider joining us as a KEIS Service Coordinator. This role involves coordinating and monitoring early intervention services for children with developmental delays within the First Steps Program.

Key Responsibilities
  1. Receive and process referrals, enter referral information into the TOTS database, and conduct home visits.
  2. Make referrals for eligibility and services, complete intake visits, family assessments, and facilitate IFSP meetings.
  3. Connect families with community resources and develop transition plans for post-First Steps services.
  4. Attend local ARC and IEP meetings as an advocate for families.
  5. Maintain regular contact with families to ensure their needs are met.
  6. Promote a culture of inclusion and participation as a positive team player.
Minimum Education and Experience
  • Bachelor’s degree in a human services field.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
Preferred Qualifications
  • Bachelor’s in a human service field and two years of experience with children aged 0-5 and their families, demonstrating skills in communication, problem-solving, organization, collaboration, and leadership.
  • Or, licensure in a recognized professional field (OT, PT, ST, DI, SW, etc.).
Additional Qualifications
  • Previous experience in service coordination or case management preferred.
  • Knowledge of early childhood development and milestones (0-3 years) preferred.
Essential Functions

Travel across seven counties in the Louisville District, conduct family assessments, develop outcomes, schedule and facilitate meetings, monitor plans, and ensure compliance with regulations. Maintain records, participate in community outreach, and foster positive relationships with families and providers.
