Job summary

Synergy HomeCare of Piscataway seeks a compassionate Home Health Aide (CHHA) to provide in-home patient care in South Plainfield, New Jersey. You will assist with ADLs, monitor vitals, and coordinate with nurses and therapists to ensure seamless care.

Responsibilities

• Provide personalized home health care services to patients, including assistance with ADLs, medication management, and other essential care activities
• Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions and coordination
• Monitor patient vital signs, report changes to the healthcare team, and implement care plans as directed
• Maintain accurate records of patient care, noting progress and changes
• Collaborate with nurses, therapists, and social workers to ensure comprehensive care
• Demonstrate commitment to excellence in patient care, following protocols and policies
• Stay up-to-date with industry developments to improve patient outcomes and care delivery
