Participates in the coordination and implementation of the hospital Chemical Dependency Program, and to provide direct services to patients and their families. Performs treatment coordination duties for assigned patients in the Program. Responsibilities include regular liaison with members of the treatment team, discharge planning and aftercare follow-up and marketing with community agencies and professionals. The Chemical Dependency Counselor will document clinical assessments and treatment plans for assigned patients and participate in the Performance Improvement Program pursuant to specifications of monitoring devices.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $23.00 to $27.02. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
