Qualifications

Responsibilities

• Observe, assess, document resident condition changes during rounds.
• Ensure implementation of care plans as directed.
• Administer medications as prescribed.
• Record physician visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Oversee and monitor aides.
• Discharge residents per facility policy.
• Complete Nursing Assessment for Physician Calls prior to contacting physician.
• Communicate with resident's contact for status updates or changes.
• Identify significant changes and take action per policy.
