Job summary

Lifepoint Health operates Palomar Health Rehabilitation Institute in Escondido, CA, seeking a Charge Nurse (RN) for Inpatient Rehabilitation. The role focuses on leadership, quality documentation, and coordination of care for medically complex patients in a 52-bed rehab setting.

Candidates must hold an RN license with at least two years in rehab/acute care, BCLS, and preferably a CRRN. BSN is preferred; compensation includes comprehensive benefits and career growth opportunities.

Qualifications

• Current state RN license.
• Graduate of an accredited registered nurse program; BSN preferred.
• Minimum of two years in a rehabilitative or acute care setting with leadership ability.
• Basic Cardiac Life Support (BCLS) required; Child and Adult Abuse Certification required; CRRN preferred.

Responsibilities

• Perform routine audits to ensure accurate and timely documentation.
• Educate and mentor nursing staff on clinical practice and documentation.
• Lead CQPI efforts and ensure patient safety and care standards.
• Coordinate staffing based on patient acuity and staff competency.
• Provide patient and family education and coordinate resources to reduce readmissions.
• Support orientation and remediation for newly hired nurses.

How You'll Contribute

• Complete routine audits to ensure accurate and timely documentation in Cerner and UDS for compliance.
• Identify and address educational needs among nursing staff based on clinical practice and feedback.
• Educate and monitor staff on proper QI documentation to reflect accurate burden of care.
• Serve as a clinical mentor and resource for nursing staff caring for medically complex patients.
• Support patient and family education and coordinate resources to reduce hospital readmissions.
• Provide UDS documentation/orientation training for newly hired licensed nurses.
• Conduct remediation sessions for nurses with documentation deficiencies.
• Participate actively in Continuous Quality and Performance Improvement (CQPI) efforts.
• Ensure the nursing team delivers care that maximizes patient safety and meets all care standards.
• Assist with nursing staff assignments based on patient acuity and staff competency.
• Perform other duties as assigned.

What We're Looking For

Applicants should have a current state RN license. Additional requirements include:

• Education: Graduate of an accredited registered nurse program; BSN preferred.
