Job summary

LiveWell Homecare Agency is seeking a Registered Nurse to collaborate with the patient care team to assess, plan, implement, and evaluate patient needs. You will serve as the primary contact for patients and families, ensuring safe, effective, and patient-centered care.

You will monitor conditions, coordinate flow, document assessments and care, advocate for patients, and participate in performance improvement initiatives to improve outcomes and satisfaction.

Qualifications

• Associate, Bachelors, or Masters in Nursing required.
• Active RN license (single state or multi-state compact).
• Basic Life Support (BLS) certification.
