Shift: 12 Hours - Night Shifts

Job Responsibilities
  • Provide care to assigned residents in a caring, safe and efficient manner according to location standards, procedures and individualized resident care plans.
  • Complete comprehensive nursing assessment of all residents on admission and on an ongoing basis according to state and federal regulations using the nursing process (assessment, diagnosis, intervention and evaluation).
  • Demonstrate competence in clinical skills as identified on Competency Verification and Training Checklist.
  • Assure assigned residents receive ordered medication, treatments and other procedures within defined time frames, using appropriate infection control procedures.
  • Demonstrate familiarity with the therapeutic effects and side effects of all medications administered.
  • Obtain equipment and supplies to provide care as needed.
  • Ensure procedures are followed for pressure ulcers, dehydration, malnutrition, incontinence, pain and medication administration.
  • Accurately complete glucometer testing and control monitoring according to procedure.
  • Implement specific approaches to manage behavior and enhance residents’ mood state according to resident care plan.
  • Assure resident care for assigned residents is completed through reviewing documentation and updating as necessary; use the EMR accurately; document actions and notations for all resident condition changes according to policy and procedure.
  • Document all actions taken related to admission, discharge, transfers or death; document incidents or accidents in the resident record and all necessary forms (incident reports, etc.).
  • Complete behavior documentation according to policy and procedure and complete seven-day observation tool.
  • Assist and participate in quality improvement processes, including audits and corrective procedures as assigned.
  • May complete assigned portions of the MDS for Nursing and sign the MDS forms to signify completion of the process.
  • Assure physician orders are processed and physicians notified in a timely manner; make rounds with physicians as assigned; attend or obtain resident report prior to beginning shift.
  • Receive and share information; observe and report residents’ conditions; respond promptly to resident call lights.
  • Direct and assist with resident, family/significant other education; notify family of condition changes; report and ensure timely follow-up of signs and symptoms of resident infections.
Qualifications
  • Graduate of an accredited school of nursing.
  • Prior work experience in a like or related position preferred.
  • Maintains required certification/qualifications and associated standards. Licensure for state of practice is required.
  • Knowledge of basic nursing skills; knowledge and familiarity with the therapeutic effects and side effects of all medications administered.
