Job summary

The Homestead of Denison is seeking a compassionate Charge Nurse (LVN/RN) to deliver licensed nursing care per resident care plans and physician orders. You will monitor residents, administer medications, document accurately, and support care coordination with the team.

PRN shifts rotate day and night; you will help with staffing, ensure safety and infection prevention, and contribute to QAPI initiatives while upholding resident dignity and family education.

Qualifications

• Nursing degree from an accredited program or valid LVN/LPN/RN license.
• Up to 1 year of charge nurse experience in hospital or facility.
• Ability to interpret resident care plans and physician orders.

Responsibilities

• Provide licensed nursing care per resident care plans and physician orders.
• Monitor resident condition and communicate updates to care team.
• Administer, monitor and reconcile medications safely.
• Complete accurate charting and required notifications.
• Support resident-centered care, dignity and trauma-informed practices.
• Assist with staffing, census accuracy, and QAPI efforts.
• Promote a clean, safe environment and infection prevention.
• Participate in care coordination and staff education.
