Job Summary:

The primary purpose of the Charge Nurse (LPN/LVN) position is to oversee and participate in patient care as prescribed by the physician, within the scope of LPN/LVN training and licensure. This role involves providing direct care to patients and supervising CNAs to ensure high-quality care.

Qualifications:

  • Graduation from an accredited Nursing School and a valid license in the applicable State.
  • Preferred: Minimum one-year supervisory experience in long-term or healthcare settings.
  • Ability to read, write, and speak English.

Essential Functions:

  • Assess and report patient conditions, document in accordance with policies and regulations.
  • Document all relevant patient information in the EMR.
  • Consult with RNs and Nurse Managers about patient condition changes.
  • Transcribe and execute physician orders securely.
  • Implement care plans, administer medications and treatments as prescribed.
  • Assign duties and oversee CNA staff care delivery.
  • Document and report incidents and accidents per policies.
  • Function as a team leader and role model.
  • Secure and maintain equipment and supplies.
  • Assist in evaluating and counseling CNA staff.
  • Maintain a safe environment for patients.
  • Notify physicians and families of significant health changes.
  • Maintain quality assurance logs and ensure staff training on care systems.
  • Enforce policies on abuse prevention and ensure patient safety.
  • Perform other duties as assigned.
